Generally it means you are not specifying the correct port for it to use.
It can play out in many ways. Typically here are a few common causes.
A. You have more than one instance of source running at a time on the same
port.
-Can be caused by not specifying the port number in srcds while having t
03 server. Load Balancing.

something of the sort at which point the bots would leave and rejoin at such
a rapid rate that the server would lag out and eventually crash. It was my
own dumb oversight and not a srcds problem though. Not sure if this helps
but a shout out none the less .
