I think there's a BIG difference between merely having the ears "put back" and 
ears "pinned flat back."   Pinned ears are a sign that something is seriously 
awry.  Ears that are partly back can mean the horse is concentrating as you 
say, thinking about something behind their head (a rider on their back, for 
one), or it may be a warning that they are considering going to full-fledged, 
pinned-flat-back ears.
