Many reasons:
1. Usually journals publish things which are properly written.
2. Sometimes they miss that too and hence the name gets invalid or
illegitimate.
3. It all depends on the reviewers about how thoroughly they can review.
Some reviewers just check technical aspects.
4. Orchid world is so big and its not possible to get all information at
one place so people miss few literature and hence they miss to understand
that the species was already identified.
5. Sometimes different people have different perceptions, some say plants
are same, some say they are different.

There are some orchids which have around 60 synonyms. As time goes by,
people realise and then they merge them together one by one.
