I need to manipulate the headers on messages before final delivery. I was
going to use a .forward file which named a pipe to a Perl script, but I'm
not sure how such scripts would name a folder.

Yes. The FAQ addresses sendmail, and you don't actually say which MTA
you're using, but it is the MTA
to do what you want already exist within your mta and mda (or
whatever the acronyms are).
