Summary: SQL DDL links to commons-csv CSVFormat, but Javadoc is
not appropriate for pure SQL docs

At this moment all the commons-csv Javadoc is 404, so the website precommit is
broken. We do not need to link to this. Our docs would be more clear if we just
copied the relevant content. We should also make our pure SQL interfaces
independent of Java library details.
