Windows 8 wants a URI with a single slash.  Here's the work-around for now:

 if (app.runningOnDevice('win32nt'))
            callback(fileEntry.fullPath.replace('//','/'), fileName);
        else
            callback(fileEntry.toNativeURL(), fileName);

Also, for those of us using AngularJS, this issue was compounded by this 
AngularJS issue:
