The issue is still there in v4.1.5
I have added two attachment showing the problem, where page 4 (auto added) is a
completely blank page. It isn't using the "Left page"-format as i should in the
case.

It seems a little ridiculous that an issue is open that long and nothing
happens.
Looks more and more like another way to say WONT_FIX, sadly... :-(
