sample doc for reproducing crash.
When the writer document has more than one embedded spreadsheet, the mail merge
feature will crash in step 6.
AOO will ask to save opened documents and restarts.
I'm attaching a sample doc to replicate bug.
Tested with AOO 4.0.1 (italian nls) and 4.1.0 AOO410m1(Build:9750) - Rev.
1560773 on Windows 8.1 64bit (italian).
I had similar issues with AOO 4.0 on OS/2, but I'm not sure it is the same bug.
