I've already read the iText FAQ and mail-archive, and I suspect the answer to my question is "no" but I'd like to make sure before deciding that iText can't help me and look elsewhere:
Can iText be used to read/parse the bookmarks (outline objects) out of an existing PDF file? I've tried to come up with ways of geting a PdfContentByte object from a PdfReader object and then calling the getOutlineRoot method, but it only appears possible to do this whith a PdfWriter object.
