Bugs show up in the strangest places. This was a side effect of
supporting pdfs over 2G, thank you all for spotting it. It will be
fixed in time for this month's release.

Instead the size of the unsigned file is: 212.622.670 byte

Short answer, you cannot use "protected" methods of the PdfWriter class
here. Check the API
I'd guess
you have more than one iText.jar file in your class path, and the one being used
lacks the AFM resources.
