I think it has to do with the length of time that the message has been sitting in your inbox, and the value can be changed under the Tools menu Andy

But I can't tell you why this happens. It might be something to do with reminders. To get rid of the red flag, while you're in the folder with the red flag message, move to that message in the messages list. Then bring up the home tab of the ribbons and find Tags and then Follow-up dropdown.
