Jim was correct. I downloaded the source for the latest stable xemacs
(xemacs-21.4.12), followed the standard install instructions, and now
the OK/Cancel buttons appear and the Import Wizard works great.
For what it's worth I ran the configure step with these options:
$ ./configure --with-widgets

I have the same problem on one machine and I
don't see a mention of --with-widgets in the
XEmacs man page
