I have noticed that it happens for me when having the following situation:
1) Two screens (HDMI + DP) attached and turned on
2) a TV (HDMI) attached and in standby
3) Boot, login and turn on TV from standby

Never mind the distinction in the steps to reproduce in my comment 7: It
