I deleted the trash folder (hadn't been cleaned up fro 2001) and manually
recreated the index file for a few large folders. This seemed to solve the
problem

Problem still exists for me in kde 4.3
Packages from kubuntu.
But don't let that fact stop you from analyzing this report :-)

Yes, I have that as well.
After startup, kmail is not completely unresponsive but might take 30 seconds
or so to react to a keypress or a click in its interface

I am on kubuntu 9.04, which is derived from Debian, but generally not as stable
