I found Kdebugdialog on Kubuntu too, but even though I turned on everything
there was no output in the file relating to KMail. I might have done something

I think it only works with a debug build of KDE or probably also if you have
installed the -debug packages (I'm not sure though).
I'm not sure if the output helps a lot.

I have installed debug packages. That does not do the trick. I will try to
contact people for further debugging, but may be it will take some time.

You can use kdebugdialog to turn on debug flags.
If I do it here (openSuse 11.1), I see e.g.
kdeinit4: preparing to launch /usr/bin/kmail
kmail(11219)
