I cannot test the issue as I do not have the laptop any more but
unfortunately the issue persits on my new laptop (Dell Latitude E5470):
the working version is 4.4.0-59.80 but with 4.4.0-62.83 and next the
problem comes back. I will post a separate bug for it.

As far as I can remember on 12.04 I did not manage to make hardware
accelerated video decoding work at all because of lack of some libraries
/ packages. I think I made it work when I upgraded to 15.10 and this is
when this issue arose.

I tested 4.7-rc1 and it gets even worse. I have not seen
usb 2-1.2: device descriptor read/64, error -110
but it stops booting and unpluging and replugging usb stick does not
make the process continue.

1) Yes, badblocks scan shows no errors
2) I make bootable USB stick from ISO using dd. I use another USB stick for
that (not Sandisk). After booting I proceed with installation process - I use
Sandisk USB as target as if it was a regular harddrive. Installation proceeds
without pro
I tested with 4.6.0-040600rc6-generic but the problem persists. Making
pause and play again in SMPlayes gives the following syslog output:

The problem was there from the first install. Previous kernels tested:
4.2.0-16
4.2.0-18
4.2.0-19
There were some errors while installing upstream kernel (I attach thefile) but
despite of that it managed to boot. The problem persists.
