Hello,
I'm suspect that this problem is similar to Bug #1562076 reported.

The Problem:
When I plug a HDMI cable to use a second screen the Ubuntu freeze, halt and I
need do a "hard power off".
This problem occurs at any kernel of Ubuntu 16.04 that I tested. Now I use a
4.4.0-21-generic #37-Ubuntu SMP Mon Apr 18.
The same hardware with Ubuntu 14 and kerne l3.13.0-85-generic #129-Ubuntu SMP
Thu Mar 17 20:50:15 UTC it's OK.
