I had this issue multiple times, but it is not always reproducable. This bug occurred now in linux-image-generic 4.4.0.53. Earlier it occurred multiple times on linux-image-extra-4.4.0-47-generic 4.4.0-47.68 amd64 Linux kernel and some earlier versions (see https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1641152). It did not appeare on linux-image-extra-4.4.0-51-generic 4.4.0-51.72 amd64 and now it appeares again (on linux-image-generic 4.4.0.53). I was not able to report it via the apport-collect.
