If you create a new customer and don't do anything else other than try to
add a note you should be able to replicate this missing dropdown issue.

You are missing a dropdown. This may be a very old template. I have
occasionally experienced code getting "stuck" in SVN and not updating
though not recently.
Try this:
rm UI/Contact/divs/notes.html
svn up
That should solve your problem by pulling a fresh copy.
