In the fields:

"Use quoted names for narratives"

The problem is that if you use the Quoted Name in narrative but
remove the Quoted name in Name lists and where the full name is used,
then there is nothing in the text that says that the person was
called by quoted name.

You should not have put names of that type in the AKA at all. Remove one
or two AKAs and then Preview the Book and you will see it is better
