Sorry Christine, I realise you posted this awhile ago. I use cause of
death, but I don't use capital letters eg leukaemia, and with my
reports I prefer sentence style for research and events, so it
displays in a narrative report something like: The cause of her death
was leukaemia.

I use the area under cause of death for any medical notes, such as illness,
surgery, etc throughout the person's life. Any notes related to the death, I
put in Death Notes.
