Tentative explanation (Please let me know if/where I'm wrong) :
- After your step 1, the image is "anchored to paragraph"
- After your step 3, the image is encapsulated in a frame "anchored to
paragraph"

Further investigation under LibO 4.0.0.0 beta2 on Win7. I confirm my tentative
explanation in comment 2. Definitivly Not A Bug. I keep the NeddInfo status, in

The problem is that it is too ... complicated. We should not need to be a
'LibreOffice' expert to be able to use basic word processing features.
So, it appears to be in fact either a conception or a default
