I had the same problem with 2.2.1.
Moreover, this issue seems to appear when another part starts a note
while the first is playing a tied one.
Is it corrected in the development branch?

I'm not sure, but -p is used for generate PDF files (it uses Type1
fonts) whereas -P generates PostScript output with bitmap fonts.
Look at your /usr/(local/)share/lilypond/1.8.0/fonts if there are "afm"
and "tfm" fonts...

Acutually there are more than 16 tracks. The problem is that the midi output
sets one channel per trac
Lilypond is a typesetter in which you have to "program" your music.
Once it is done, you can launch "ly2dvi file.ly" in order to obtain a
dvi output.
If you want a graphic interface (a GUI), you can try Rosegarden
