Are the intructions in NR 2.2.1 Cross-staff stems, inadequate?
This doc section was rewritten very recently, so I'd hope that the docs
are correct on this point.

Thanks, that is perfect. I was missing "\override Stem #'cross-staff
= ##t", which has presumably been added for 2.11.
