This seems to happen if the hardware rejects a TRB with status TRB_ERROR (5).
In this case the 'address' field is that of the broken TRB not that
of some dma buffer.
While the TRB_ERROR status shouldn't happen, the

unplugging the usb3 cables silenced it. plugging back in, everything
came up fine.
