You should explain what you're trying to achieve by these changes.
The EHCI changes look like trouble, as does the usb-storage one.
The hub changes might be better, but I didn't look at them. I know
there are at least refcounting problems though that code path.

present.

yes you are right. I use a hard disk in PIO mode. I will use a flash disk
also.
I can reproduce the problem the same way as you can.
I think it is probably in the USB layer, but I am not sure, if the pwc could
not handle
the problem better than it does. I must not reload the pwc driver,

Do you have any USB devices plugged
When kernel is mounting root file system it is doing it too fast so
usb-support have not ime to scan bus for mass-storage devices and
connect them.
i've tested bios usb support, so i've checked how fine different OSes
boot from USB mass-storage divices. here is my linux test:
