My server has this line in /etc/inetd.conf:

tftp            dgram   udp     wait    nobody  /usr/sbin/tcpd  /usr/sbin/in.tftpd 
/tftpboot

So, this is what happens:

ambapali@tanatos ~$ tftp localhost
tftp> get /tftpboot/lts/vmlinuz-2.4.18-3c905c
Received 1117242 bytes in 0.9 seconds

I'm running Debian 2.2 on the server with tftpd 0.10-1 that does not accept -s
flag (must be an old server).  The file /tftpboot/lts/vmlinuz-2.4.18-3c905c is
readable by everyone (# chmod a+r /tftpboot/lts/vmlinuz-2.4.18-3c905c) and the
directories /tftpboot and /tftpboot/lts must be accessible (executable) by
everyone.
If your version of tftpd can run chrooted, that is, it takes the -s flag, than
you should ask it to load the /lts/vmlinuz-2.4.18-3c905c file, you will not
have to specify the /tftpboot directory.
