I've been playing with the same export. So far I've been really happy. Rather
than using Calibre to do the conversion, you might take a look at Sigil. It
creates a much cleaner ePub (though Calibre is more than an adequate
solution).

The only way I can think of is, unfortunately, through code, or ERT (Evil
Red Text).
In your document, press CTRL+L, and a red box will appear. After your front
page you could tyoe in the red box : \pagestyle{fancy}

like appear in the upper left corner of the second screen. I would rather
have them on the primary screen where I work with Lyx. Is there a way to
influence this
