Have you tried to turn off voice-over and then back on? Sometimes this clears issues like this. I have found that Voice-Over and commands sometimes do not work and you get a ding sound. After I toggle voice-over it works again. Cannot reproduce the issue on will, thus have not reported it.

This is why I don’t like the VO approach to web pages. All other approaches treat the page as a document. Thus you just need to use your arrow keys to navigate and not interact with frames, tables, etc. Simple pages in safari work fine. It becomes real difficult when you have a complex pages.
