Thanks everyone for the suggestions. I do not use the trackpad. I will start
taking more notes on when the issue occurs and does not.
I do not want to go through the process of rebuilding the laptop. Since it is
my work laptop and will cause me major headaches.

but my money is on a software issue that isn't universal to all VoiceOver

It almost sounds like a hardware problem of some kind, i.e. sticking keyboard
keys. I have been using ML for months, both in beta testing and regular
day-to-day use, and never ran into this problem once.. I wish I had other
suggestions to offer, but at this moment I'm rather stumped.

I've experienced this issue regularly in both Lion and ML but have not been
able to determine a cause.. In my case, the standard duct-tape solution (VO
off, VO on) straightens it out for a while.

I've never seen anything like this before. It almost sounds like your mouse
cursor is set to move the VoiceOver cursor, which it might do if you
accidentally brushed the trackpad. I can't think of anything else which could
be causing this.
