It's actually cmd d not cmd shift d.
the cmd option b and cmd shift b are to do with the book marks table and tool
bar.
After you hit cmd d simply choose the book marks collection you want it to go
into.

How can a create a simple book mark for a webpage? I've tried Command shift B,
but VO just says hide [or show] book marks. Looking under the bookmarks menu,
