VO not seeing all of the screen?

Hi all - I was trying to create a dmg passworded folder using Disc Utility. All was going ok until I got to where I was asked for a password. Somehow, the screen changed, and all VO would read was: Close [dimmed] 0 percent. Cancel [dimmed] and creating. Apparently however, Also visible on the
