Got it fixed. My wife had to: 1. turn vo off; 2. launch iTunes for me; 3. play a track; 4. turn volume up.
When no sound was coming out of the device, i.e., nothing was playing, using the buttons had no effect. It brings up an interesting one though: should VO volume not be separate, or seperable from the main system volume? Also, perhaps a gesture should be included to bring volume up a bit?
