I do the same: close everything before shutting down. I really hope they're 
able to sort out this hit and miss startup of
Voiceover. I know hitting command tab isn't a huge deal, but it still bugs me 
that its necessary. Snow Leopard didn't have
this issue, and I don't have anything that strange happening on my windows 
system. This started with the first verssion of
lion with that com.doc.apple.extra thing, and they cleared that up, but the 
underlying problem still remains. With any luck,
they'll actually fix it in the next lion update, or do you think we'll have to 
wait for Mountain lion and hope for better
things then?

The slow startup without speech is a separate problem. Someone posted on the 
list that it's a bug, and the results are the
same for me whether I checked that box or not.

Most of the time I just close everything before logging out.
