So what worked for me was John’s excellent suggestion of going to the previous
recipients window and clearing everything from there.
Seemed to be the easiest way to resolve the problem and it worked.

I know you can rebuild a mailbox database,
Can you not do the same thing with an address book database?

hardware hahaha. I think the sort and clear method is in order.

Go to windows and open up previous recipients. I then usually sort by last used
and check a few to ensure they have been added to contacts. After that I clear
the entire table.

Just to add to this, I've also seen this. It only happened once, but somehow
the phone number for our vet was added as a second phone number to an entry I
have for a local pizza place. I noticed it on my iPhone, did not check whether
that carried over to the Mac as well. I went into Contacts
