Scott, Have you cleared out your previous recipients list? Go to windows and open up previous recipients. I then usually sort by last used and check a few to ensure they have been added to contacts. After that I clear the entire table. This is something probably worth doing a few times a year

Just to add to this, I've also seen this. It only happened once, but somehow the phone number for our vet was added as a second phone number to an entry I have for a local pizza place. I noticed it on my iPhone, did not check whether that carried over to the Mac as well. I went into Contacts
