I never used to be able to get chapters to work with AudiobookBionder until I
found the following tutorial:
http://bluezbox.com/audiobookbinder/chapters.html
Just follow its instructions exactly. You can also access all of those
chapter-related commands in the Chapters menu in the menu bar.
