I would have a headset or speakers plugged it as backup. You can go under VO
utilties and control what output VO can come out. For example, If you are
watching a movie on your tv, you can just hear VO on your headphone.
Just an idea. Also there is a way to mute VO speech too for trackpad usage y
I have a mini hooked up to a monitor with a separate speakers plugged into the
jack on the mini. This allows me to control the volume with either the knob on
the speakers or the software volume control.

called the Logetech H800 headset. It has a microphone as well as the
headphones. It can be used both as a bluetooth, and also has a dongle support,
which means you plug this tiny receiver thing into your USb

I just had this happen yesterday and I'm still running Snow Leopard. A reboot
fixed it. I was playing Audio Quake at the time and VoiceOver was *NOT* on.
Guess it'll still be a potential in Lion.

If your headphones are less then one year old, take them to an Apple
store and they should replace them.
