I have found that if I invite someone to join a list, and they reply via
email to activate, my maillog looks like this:

Arrgh.
It was a problem with postfix-to-mailman.py. I had {mailbox} not {user} in
my main.cf... the version of p-t-m.py I had, had said use:
mailman unix - n n - - pipe
flags=FR user=mailman:mailman
argv=/usr/local/mailman/postfix-to-mailman.py
