I know that the same thing happens when you use 'clear cosmetic objects'
command. The untouched autolabels are removed but not the labels that have been
manually repositioned.

Frank, you are correct -- once you move a label, or edit it in any way,
it is no longer an autolabel. There is another layer of control you
have over labels though. In the layer control's Label button, you can
