Michael, I can think of at least two possible culprits.
One, If one end of the line is overweighted with nodes, this can pull the direction arrow to the end and hide it. Two, and this one stumped me for some time, the line that is missing the direction arrow could be a member of a multi-segment ployline. In other words, at some point it's possibe that this and one or more other plines whose end nodes were not snapped, were joined or combined creating this multi segment line. If this is the case, you will need to either "explode" this line using a poly line exploder MBX, or rebuild it (you'll also have to rebuild the other members too. The remaining part(s) or members of this pline could even be far enough out of zoom range thus you never notice it when this line is selected.
