Usually this means that everything is OK but you are somehow asking image from
an area that is not covered by your image. Tell us
- the EXTENT in your mapfile
- the PROJECTION block of the mapfile at MAP level
- the request you are using for getting the image
Add DEBUG 5 into your mapfile
