Currently we hold our company procedures and process on a wiki. Our
other systems have links to the relevant pages in the wiki. Thus our
inventory system might have a link to standard text that needs to be
sent out. EG an example is:-

I've always used the reinstall-technique when upgrading my wiki, but I've never
user the /maintenance/update.php but rather the alternative described
in
http://www.mediawiki.org/wiki/Manual:Upgrading#Alternative_2:_Re-run_the_installer
