Holy crap, I didn't realize you could do Special:EditUser/Username!
The only thing that I could suggest is checking permissions on
extensions/EditUser/ (and its contents.)

I use (and like) http://www.mediawiki.org/wiki/Extension:EditUser, but
it stopped being maintained and may not work on > 1.15.

If it's not there, subpages aren't actually enabled. They /kind of/
work, but a lot of weird things happen. It's been a while, but I think
this exact thing happened to me.
To enable it for that namespace, look into the $wgNamespacesWithSubpages
variable in LocalSettin

syntax and specifying arguments, you can get it to list
articles as well, but nothing can be done to combine them.
The example you've given would just give two separate trees:
[−] Example 1
Example 1 Page 1
Example 1 Page 2
Example 1 and 2 Page 1
[−] Example 2
Example 1 and 2 Page 1
articles in one category, but listing all

This can be done with
http://www.mediawiki.org/wiki/Extension:Multi-Cat
to prevent it from being included in any transclusion of
that source.
From your description of what you're looking for, I'm pretty sure what
you're looking for is [[Template:NameOfTemplate]].
