What are the permissions and ownership of LocalSettings.php? Run "ls -l" and see what it shows. Do you know if SELinux is turned on and enforcing? That can make files appear to be missing to Apache. Try running "sestatus" or "cat /etc/sysconfig/selinux" and see what they tell you. If it is, th

The results of your ls show LocalSettings.php, so my first guess would be that your server is not set to use that specific directory as source for your wiki content. As a first step, you may want to check both index.php and LocalSettings.php in that directory to make certain that both are readable
