(YENTI - mama VARI) "KATANYA 'FLU', TAPI KOK BUKAN FLU?" Kata flu ternyata banyak "dibajak". Orang begitu gampang mencantelkannya pada nama penyakit tertentu yang ternyata bukan masuk kategori flu biasa. Pernah dengar, kan, orang menyebut flu Hong Kong, flu Singapur, flu perut, flu tulang, flu burung dan masih banyak lagi. Menurut Dr. Alan R. Tumbelaka, Sp.A (K), flu yang benar-benar flu dikenal dengan sebutan influenza. Ada pun penyebabnya adalah virus influenza. Penyakitnya juga relatif gampang diatasi. Setelah diberi obat penurun panas, antibiotika jika perlu dan obat batuk jika diikuti batuk serta istirahat cukup, kondisi tubuh penderita umumnya akan kembali pulih. Kalaupun berbagai penyakit tadi diberi embel-embel kata flu, tegas konsultan Penyakit Infeksi dan Pediatri Tropis Bagian Ilmu Kesehatan Anak FKUI-RSUPN Cipto Mangunkusumo, boleh jadi penyakit tersebut memang memiliki gejala awal seperti flu. Memang hampir semua penyakit akibat virus memiliki gejala mirip flu. Yakni diawali batuk-pilek, demam/panas tinggi hingga penderita menggigil, sakit tenggorokan, otot terasa pegal-pegal, sakit kepala, batuk kering dan biasanya disertai mata merah. Tak heran begitu ada penyakit lain yang memiliki kemiripan gejala awal, lantas orang mencantelkan kata flu di depannya. CARA MENEGAKKAN DIAGNOSIS Setiap jenis virus pada masing-masing penyakit memiliki tingkat keganasan yang berbeda. Kalau virus influenza jelas mudah dijinakkan, namun untuk flu-flu lainnya belum tentu serupa. Apalagi bila ditengok sifat genetisnya, virus flu dikenal sebagai virus yang tidak stabil. Ia sering bermutasi akibat pertukaran gen. Tak heran kalau sering muncul jenis flu-flu "baru" yang sulit sembuh, lebih parah gejalanya, bahkan sampai menelan banyak korban. "Contoh terbaru adalah SARS yang katanya disebabkan corona virus. Virus ini sebenarnya sudah lama dikenal di dunia medis. Hanya yang sekarang ini adalah jenis terbaru dari corona virus sehingga lebih ganas sampai memakan banyak korban meninggal." Yang sering jadi pertanyaan, bagaimana penderita tahu persis virus apa yang tengah berkecamuk dalam tubuhnya. Terlebih bila semua penyakit yang disebabkan virus memiliki kemiripan gejala. Menurut Alan, menegakkan diagnosis suatu penyakit yang disebabkan virus memang bukan perkara mudah. Pasalnya, untuk membiakkan suatu virus dibutuhkan waktu lama dan teknologi tinggi yang di Indonesia kadang-kadang belum tersedia. "Jadi, jangan heran kalau untuk memastikan seseorang terkena virus SARS atau tidak, sampel air liur penderita sampai dikirim ke Amerika." Itulah mengapa biasanya dokter akan mengobati penyakit berdasarkan gejala yang timbul. Cara ini sering disebut dengan pengobatan simptomatik. Namun ini pun tak mudah. Seorang penderita dengan gejala flu, contohnya, akan diduga menderita influenza. Padahal belum tentu penyebabnya virus influenza karena untuk memastikan virus tertentu butuh pemeriksaan lama. Akhirnya, dokter hanya mengobati gejalanya saja. Kalau demam diberi obat antidemam, sedangkan jika disertai batuk akan ditangkal dengan obat batuk. Namun karena jenis virus yang menyerang tidak diketahui secara pasti, maka biasanya penderita tidak diberi obat antivirus. Apalagi sifat virus umumnya susah dimatikan, sehingga si penderita susah diobati. Makanya, ujar Alan, dokter kerap tak mau ambil risiko memperparah penyakit yang diderita pasien. Akibatnya, tak sedikit dokter yang lantas menembaknya dengan antibiotik. Dengan asumsi mungkin penyebabnya bukan virus, melainkan bakteri. Bukankah bakteri akan mati jika dihantam dengan antibiotik? YANG PENTING STAMINA Lalu apa yang bisa dilakukan untuk menghadapi berbagai kemungkinan penyakit tadi? Saran Alan, ketimbang kelewat mengkhawatirkan sekian banyak macam penyakit maupun pengobatannya, lebih baik jaga stamina alias daya tahan tubuh agar tetap prima. Soalnya, seseorang hanya mungkin terjangkit penyakit bila daya tahan tubuhnya melemah. Minum vitamin C yang banyak didengungkan memang merupakan salah satu cara membantu meningkatkan daya tahan tubuh. Yang tak kalah penting, lantaran penyakit yang disebabkan virus cepat menular, maka penderita yang sudah telanjur terkena harus sadar diri untuk segera memutus rantai penularan. Solusinya dengan beristirahat di rumah. Ini penting bagi penderita sekaligus bagi orang lain di lingkungannya agar tidak tertular. Jangan lupa gunakan masker sehingga virus yang bisa disebarkan melalui udara maupun percikan air liur tidak "lari" ke mana-mana. Vaksin Influenza Khusus influenza yang disebabkan virus influenza, menurut Alan, memang ada tindak pencegahan berupa vaksinasi. Hanya saja vaksin tersebut akan berubah setiap tahun tergantung hasil penelitian yang dilakukan para ahli. Contohnya, sewaktu virus Hong Kong mewabah, maka di tahun itu pula dibuat vaksin untuk melawan virus tersebut. Namun karena tahun berikutnya jenis virus berubah, vaksin yang diproduksi pun berubah. Mau tidak mau yang bersangkutan perlu divaksin kembali di tahun-tahun berikut. Soalnya, penyebab virus influenza akan berbeda-beda setiap tahunnya. Mungkin saja virusnya masih sama, namun bermutasi lalu membentuk suatu jenis virus baru. Tentu saja tidak semua orang wajib mendapatkan vaksinasi influenza. Yang dianjurkan adalah mereka yang memiliki penyakit tertentu yang bakal lebih parah bila ia terserang influenza. Penderita asma, contohnya, akan mengalami keluhan sesak napas yang parah bila terkena flu. "Sayangnya, orang masih sering salah kaprah. Mereka beranggapan kalau sudah disuntik vaksin influenza pasti tak bakalan terkena batuk pilek. Padahal kalau ia diserang batuk pilek yang disebabkan bakteri tentu tidak bisa dicegah oleh vaksinasi influenza ini." Meler Karena Alergi Mengingat gejala flu yang akrab ditemui adalah batuk pilek, Alan mengingatkan jangan gara-gara batuk pilek sedikit saja lantas cepat-cepat minum obat flu. Boleh jadi penyebabnya bukan virus influenza, melainkan alergi terhadap sesuatu, semisal dingin atau debu. Sering dengar, kan, keluhan para orang tua, "Anakku enggak demam, tapi, kok, hidungnya meler terus, sih." Sekadar Nama Berikut beberapa penyakit yang menggunakan kata flu, namun sebetulnya bukan influenza. FLU SINGAPUR "Flu" jenis ini sempat menggemparkan dunia beberapa tahun lalu. Padahal penyakit yang sering disebut dengan HFMD (Hand-Foot and Mouth Disease) atau Penyakit Mulut, Kaki dan Tangan (MKT) ini sebenarnya merupakan penyakit kulit. Kalaupun orang menyebutnya sebagai penyakit flu, tak lain karena gejala penyakitnya hampir sama dengan influenza. Penyakit yang disebabkan virus Coxsackie 16 ini sebenarnya merupakan penyakit yang umum terjadi pada anak dan bayi. Terlebih karena virus ini bisa menyerang sepanjang tahun. Selama 1-2 hari pertama ditandai dengan suhu badan meninggi, batuk pilek, mual-mual, dan tak nafsu makan. Kemudian muncul gejala bintil-bintil atau visikel seperti cacar air di daerah tangan, kaki, dan selaput mukosa mulut serupa sariawan. Bintil dan lepuhan berisi cairan bening yang mengandung virus ini sangat mudah menular, baik lewat percikan ludah saat bersin, sentuhan dengan bagian tubuh yang berbintil tadi ataupun penggunaan kebutuhan rumah tangga secara bersama seperti handuk. FLU HONG KONG Disebut juga dengan flu burung (avian flu) yang muncul sekitar Maret 1997. Jenis flu ini awalnya menyerang unggas di Hong Kong. Namun 2 bulan kemudian juga menyerang seorang bocah laki-laki berusia 3 tahun di Kowloon yang akhirnya meninggal. Setelah diteliti, ternyata penyebabnya tak lain adalah virus flu burung tersebut. Di akhir tahun 1997 muncul pula infeksi dengan komplikasi berat seperti pneumonia dan ensefalitis (radang selaput otak). Kalau tidak segera ditangani, infeksi ini bisa berakibat fatal. Pemerintah Hong Kong akhirnya memutuskan untuk melakukan pemberantasan besar-besaran dengan memusnahkan semua unggas yang dijualbelikan di pasaran. FLU BABI Ditilik dari namanya, virus penyebab flu yang juga disebut sebagai swine flu ini berasal dari babi. Sumber penyebarannya diperkirakan berasal dari sebuah tanah pertanian di daerah Midwest, AS, tanah peternakan babi. Banyak ahli memang percaya bahwa sebelum sampai di tubuh manusia, semua gen virus flu memiliki tempat-tempat persinggahan sebagai vektor penyebarannya. Salah satunya yakni melalui hewan. FLU TULANG Umumnya semua gejala flu/selesma atau common cold muncul karena infeksi berbagai jenis virus. Dari sekian banyak virus penyebab flu, ada yang menghasilkan toksin/zat racun bagi tubuh. Zat racun ini kemudian menyebabkan berbagai gangguan fungsi pada sistem tubuh individu yang terkena. Di antaranya pada sistem otot rangka. Selain membuat tubuh yang bersangkutan memberikan reaksi radang, di antaranya berupa demam dan nyeri otot dan tulang serta memunculkan warna kemerahan pada mukosa yang mengindikasikan melebarnya pembuluh kapiler di bawahnya. Berdasarkan gejala itulah, beredar istilah flu tulang. FLU PERUT Seperti telah dikatakan, toksin yang dihasilkan oleh virus penyebab flu tadi akan menyebabkan berbagai gangguan. Termasuk gangguan terhadap fungsi sistem pencernaan yang antara lain ditunjukkan melalui gejala mual, muntah, diare dan mulas. Atau bisa pula mengganggu fungsi usus, hingga pencernaan jadi tidak sempurna dan menghasilkan banyak gas. Gejala-gejala tersebut belakangan sering disebut sebagai flu perut. Faras Handayani. Hand-Foot-Mouth Disease Related concepts: Coxsackievirus A16 Introduction: A miserable child who has lost interest in eating solids? Perhaps the child has hand-foot-mouth disease. What is it? Hand-foot-mouth disease is a common childhood illness featuring mouth sores, fever, and a rash. Similar-sounding "foot-and-mouth disease" is a disease of cattle and is not related to this condition. Hand-foot-mouth disease is usually caused by a virus called coxsackievirus A16. However, many children with coxsackievirus A16 infections do not have all of the features of hand-foot-mouth disease. Some have no rash, some have no mouth sores, and some even have no fever. A variety of other viruses in the Enterovirus family can also cause hand-foot-mouth disease (the coxsackieviruses are enteroviruses). Who gets it? This disease is most common among young children but is seen with some frequency up until puberty. Adults can get it, but this is much less common. Once people have had coxsackievirus A16 they are generally immune, but they could get another case of hand-foot-mouth disease from one of the other, less common viruses. Most infections occur in the summer or early fall, with the peak between August and October in the northern hemisphere. What are the symptoms? Children with hand-foot-mouth disease usually start to feel crummy 3 to 7 days after they were exposed. Often, the first thing parents notice is their children's decreased appetite for solids. Children may also have a fever and a sore throat. A day or two later, many children develop sores in the mouth. They begin as small red spots on the tongue, gums, or mucous membranes. They may blister or form ulcers. A skin rash may also develop over a day or two, with flat or raised red spots. Unlike with many rashes, the spots are often found on the palms and soles. Also, it is common to have the rash on the buttocks. Often, the red spots will form tender blisters (although not on the buttocks). Unlike with chickenpox, the rash does not itch. Usually the rash disappears and the child feels better within about 1 week. How is it diagnosed? Usually the diagnosis is made based on the history and physical exam. Lab tests are available for the coxsackieviruses and other enteroviruses, but they are not usually necessary. Hand-foot-mouth disease is sometimes confused with strep throat, which can also begin with a fever and sore throat. It is sometimes mistaken for chickenpox because they both have blisters. It might be confused with any of the childhood exanthems. Also, many children with hand-foot-mouth disease are diagnosed with ear infections because the eardrums may appear red. How is it treated? Antibiotics do not help with hand-foot-mouth disease. The important issues are pain relief and plenty of fluids. How can it be prevented? The viruses that cause hand-foot-mouth disease are present both in the stool and in the respiratory secretions. It can spread by fecal-oral transmission, droplet transmission, contact transmission, and by means of fomites. Hand cleansing-especially after diapering/toileting and before eating-can help reduce its spread. Children are often kept out of school or daycare for the first several days of the illness, but it is not clear this prevents others from becoming infected. Other children in the class are probably contagious even though they will never develop symptoms. The purpose of this brochure is to provide helpful tips on how to care for your child or children with Hand Foot and Mouth Disease (HFM). We understand that you are very worried and concerned, and we would like to share some useful tips on how to care for your child at home. This way, we can prevent complications like dehydration from refusing to drink fluids. 1. Incubation period and expected course of HFM The incubation period after contact varies from 2 to 6 days, and your child's fever and mouth discomfort are usually gone by Day 3 or 4. The mouth ulcers resolve by Day 5 to Day 7 of the illness, but the small water blisters or red spots on the hands, feet (and occasionally the buttocks and limbs) can last up to 10 days. Sometimes, your child may complain of pain over the blisters or spots located on the palms and soles. Your child's fever be as high as 39oC. 2. How to care for your child at home: a) If your child's mouth ulcers are painful, you will notice drooling and feeding difficulties. You can give your child syrup Panadol (the actual drug name is"acetaminophen") to relieve the severe mouth pain aswell as the fever. We suggest that you serve the syrup Panadol 30 minutes before each main meal (breakfast, lunch, and dinner). It is safe to serve panadol on an empty stomach. Remember to serve Panadol only at 4 or 6 or 8 hourly intervals. If your child is allergic to Panadol, please inform your paediatrician immediately. Please seek your paediatrician's advice if you intend to serve other drugs like Voltaren or Voren (diclofenac) suppositories or oral ibuprofen syrup. These prescription drugs are stronger than Panadol and may not be suitable for some children. b) If you are self-medicating or giving your child cough mixtures or oral antihistamines, please inform your paediatrician. Some children are sleepy after drinking cough mixtures and antihistamines, and this may cause drowsiness. We suggest that you stop serving cough mixtures as your drowsy child will drink less fluid and this may lead to dehydration. Drowsiness in your child may be an early sign of brain infection as well. c) Encourage plenty of clear fluids and change to asoft diet. Younger children with painful mouth ulcers find it difficult to suck on bottle teats - we suggest that you spoon-feed your younger child. Some children like to drink from straws or cup. You can serve infants and young children home-made porridge water, barley water or chrysanthenum tea. Another alternative is half-strength flat lemon-lime soda like 'Sprite' or '7up', served at room temperature for children above one year of age. Do not serve 'Sprite' or '7up' to infants below one year of age! Remember to stir the soda until the fizz is gone because the bubbles can bloat up your child's tummy, and then add clean drinking water to dilute the Sprite or 7up. Avoid giving your child sour, salty or spicy foods and avoid foods that need much chewing. Soft jellies, melted ice cream and mashed potatoes can be offered to "fussy eaters" in addition to the usual diet of porridge or bread. d) Encourage your child to drink more fluids so that they will "pee" at least every 3 to 5 hourly. We suggest that you wake up your sick child at 2 or 3 am in the early morning to measure his or her temperature, serve the panadol if necessary and encourage your child to drink water or fluids before he or she returns to sleep. It is important to prevent dehydration in your child. e) How to apply mouth gels: Use a clean piece of gauze or clean cotton towel to dab the mouth ulcers, before applying the mouth gel. If you do not gently wipe away the saliva, the gel will not stick to the ulcer! The best time is to apply the mouth gels before meals and before bedtime. You can purchase Medigel or Bonjela from most pharmacies without a prescription. Do not share the mouth gel with other children. f) Wash your hands thoroughly with soap or disinfectant after you have wiped your child's bottom. The virus is present in stools and all body fluids. Do not allow your child to share towels, toothbrushes, cups or bottles with family members, and remind child to wash his or her hands. After the blisters and ulcers have disappeared, do not let your child participate in active sports for two to three weeks after the HFM illness. Please contact your paediatrician or return to the hospital if: 1. If your child has not urinated or 'pee' for more than 8 hours. 2. Your child starts to act very sick (looks dull, drowsy, pale or mottled, restless or irritable and unable to get into sleep, or develops cold sweating). 3. The fever lasts more than 3 days. 4. The mouth pain becomes severe. 5.
