what does happen when you change all tracktype=grade4 to grade3 or grade5?
That is, put a line like
tracktype=grade4 {set tracktype=grade3}
(I hope I remember the syntax correctly...)
in your lines file, rather near the top of the file.

the problem is in mkgmap. I can reproduce it with the attached simple example.
I think the bug is in the distance calculation in isPointInShape(), see
