looks like the httpd you are running does not have Apache::Util linked in.
EVERYTHING=1 should take care of that, which you can see using nm:
% nm ../apache-1.3/src/httpd | grep Util
0809b800 T XS_Apache__Util_escape_html
0809b750 T XS_Apache__Util_escape_uri
0809b8a0 T XS_Apache__Util_ht_time
