Did you actually "make install" in the mod_perl source tree ?
My configure line is:
./configure --enable-perl-glue
--with-apache2-apxs="/usr/local/apache2/bin/apxs"
--with-perl="/usr/local/bin/perl"
I use this typically
./configure \
--prefix=$SOFTWARE/apreq/2.08/5.8.8-2.2.4-prefork-2.0.3 \
the trees etc.
