Hi,
%= check_box "some-name" => (class => "form-control")
should do exactly that.
You can check it with this one-liner:
$> perl -Mojo -E 'a({inline => "%= check_box \"some-name\" => (class =>
\"form-control\")"})->start' get /
Returning:
<input class="form-control" name="some-name" type="checkbox">
