[Muscle] Setup method and APDU documentation

While almost all of the apdus of the cardedge are well documented the setup apdu seems to be left out. I currently have some problems with muscleTool and xcard on windows. So I have analyzed the apdu and the code and documented both. If some developer of the applet reads this you could perhaps
